Forms:

2. Transcript of Records Form
3. CV example
4. Letter of Acceptance example
5. Final changes to Learning Agreement

A student applying for traineeships at the University of Bielsko-Biala adds the following documents to the compulsory Forms:

1. copy of ID document / passport
2. copy of visa (if applicable)
3. copy of health insurance valid for all period of journey and staying

and sends them via regular post along with the applications or at least via e-mail before arrival.

In order to extend your study/traineeship period please contat your Coordinator at both the University of Bielsko-Biala and at your home university.
